By signing up for Team Wellness through American Screening Association, Inc., you agree to respect all participants within the group. Refrain from topics political in nature or any controversial issues. Always keep any form of information concerning other team members confidential and private under any circumstances. Refrain from any negative or derogatory comments regarding any member of the group during group sessions or in communications with other members. By submitting this form, you acknowledge that the services offered do not represent fitness training, psychological counseling or any kind of therapy or acting as a Nutritionist of Dietician. Understand you should consult a physician if warranted before engaging in any physical activity. As well as, aware that coaching results can vary and are not guaranteed. Agree that you are entering into coaching with the understanding that you are responsible for your own decisions and results. Agree to hold the Coach free from all liability for any actions or results for adverse situations created as a direct or indirect result of advice given by Wellness Coaching or Team Wellness. You acknowledge that you have carefully read these Terms and Conditions and fully understand that it is a release of liability. You expressly agree to release any employee or representative from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against them or American Screening Association, Inc.
